Let’s walk you through your first visit!

When you arrive at our office, our receptionist will greet you and ask if you have completed our Patient Forms. These can be found on this website under the “Patient Forms” section. If you are unable to fill out the forms prior to your first visit, please arrive 30-minutes in advance of your appointment to complete our patient intake process in-person.

Next you will meet with your doctor to undergo an evaluation and discuss your treatment options. A current digital radiograph will usually be a part of this process. Other tests may be necessary in certain cases to assist in diagnosis. After deciding on treatment, you will receive instructions on how to prepare for your procedure.

Financing options will be discussed with our treatment coordinator. Should your insurance cover part of your treatment, we will assist in submitting the appropriate forms so as to maximize your coverage.
